I'm not totally aware of the changes that Apple did when they went from 
9.0.4 to 9.1, but I have found that it (9.1 and above) does not play 
nice with the rest of the items in the system folder, especially not 
doing a clean install.  The least problematic installation of 9.1 and 
above that I have experienced is to do a clean install of the system, 
then reinstall any of the peripheral drivers, then move over the other 
items from the Previous System Folder that I still needed.
