Avid sold video digitizing and deck control cards with their viodeo 
editing software. They list a bunch of Meridian cards on their web site 
with similar-looking ID's. I expect that's what it is.

I couldn't find that one on their web site, but a call to them may ID it.

Could be worth anywhere from very little to quite a bit.
