OK, I went and pulled out my old 1212. I have been using my UD-0608-A's as
they don't take up so much desk......

You will not need #3

Getting a pen is easy, just buy a wacom (4x5) artpad off eBay that has a good
pen. I found some a few years ago for about $30-35. Replacement pens are $50
(if I remember correctly) from Wacom.

My UD-1212-R has a Mac serial connector at one end of the cable and the DB-9
at the other end for connecting to the tablet. The power supply plugs into a
small cord coming off the Mac serial connector (poor design). I got one a few
years ago from Wacom, as I bought a PC 1212 on eBay and was able to get a cord
for use on a Mac. I do remember the first one they sent had an intermittent
short where the power cord went into the serial port. I had to have them send
another (I might still have the first one if I can find it). Might also have
the old PC one too.

You might check via email with someone at Wacom to see if you can get one
along with a power supply. Mine states.....
