This looked fun so I tried it out. Here's what I came up with:

1) Use DiskCopy to create and mount a new DiskImage of whatever size
you need.

2) Use BBedit (or whatever) to create your Start Here! html file, save
it to the mounted image.

3) Use the Terminal.app to create a invisible directory/Folder. I
called mine .graphics. The . tells OSX that the Folder is invisible.

[macison:~] scoog% cd /Volumes/"your DiskImage here"/
[macison:/Volumes/your DiskImage here] scoog% mkdir .graphics

Again, I used Terminal to copy my images into the invisible Folder:
[macison:~/Pictures] scoog% cp cross.jpg /Volumes/your\ DiskImage\
here/.graphics/cross.jpg
[macison:~/Pictures] scoog% cp Reed.jpg /Volumes/your\ DiskImage\
here/.graphics/Reed.jpg
[macison:~/Pictures] scoog% cp dot.pict /Volumes/your\ DiskImage\
here/.graphics/dot.pict

I think you could use Finder in OS 9 to put a Folder on the Disk and
the use ResEdit to make it invisible.

4) Instead of frames use the target="_parent" tag to keep the "images"
opening in the same window, example:

<a href=".graphics/asax.gif" target="_parent">Your Button</a>

5) Now adjust all the window presentation features you would like to
include:
Make the Icon for Start... the Size and image you'd like.
Drop in a background image (my fav trees.jpg) and make the file
invisible.
Adjust the window size, shape, background image etc.

6) Unmount the DiskImage and Burn to it to a CD
