I use a 4 port USB/FW combo card and have had only a few problems.  Have a
HP printer and a Firewire CD-RW connected.  You need to install the Apple
USB and/or Firewire support *prior* to installing the card and be certain
you have the right card-(USB 1.1 or 2.0) for your system.  Make sure the
card is properly seated and you reset the cuda button.  On one system, had
the same problem, had to pull the card, re-install the Apple USB support
1.41, and reset cuda button, then all worked properly.
