On a four wire Molex power lead, one red, two
black, one yellow, is it 12 v red, 5v yellow, two black grounds?

Generally, (in computers) red is 5V, and yellow is 12V. You can't always count on this on, e.g., motherboard power supply connectors, but on the hard drive connectors it is a pretty reliable rule. And yes, the black wires are ground.

Orange tends to be 3.3V, but those aren't present on the hard drive connectors.
