I'd guess the adaptor? I use a programmable one with
little switches that give different resolutions to run a Sony
tube that has 5 bayonet mounts on back and the quintopus cable
goes from these to a vga plug. The vga via my adaptor to my 7300
(used to be 7600) and 1024 x 768 at about 75 MHz. One such
adaptor - not being used at moment so I can see it on my desk -
has 2 rows of 8 switches and has a great range of resolutions.
It is a Unimac from total technologies (lovely little thing made
in 1993, remember that century?)
