Tom, I use the sample workflow, but have never seen this problem. Actually, we have not had any problems with our files at the printer or prepress shop that makes our proofs. Since the pdf's you supplied came from 3 different sources(This doesn't sound good for other reasons), it sounds like there could be a problem at the prepress house or while files are sent. Did you try sending the same files on a CD? Also, is the banding found only in the images? Did you re-distill any of the files and get them to work?
