I must convert hundreds of pages of HTML (created with Dreamweaver 4) into
PDF "books", and the users of those books require a Table of Contents
complete with page numbers. 

I was told that Acrobat v6 would do this (which is why we purchased the v6
upgrade), but I've since been told that Acrobat _won't_ generate a ToC with
page numbers! 

I tried using HTMLDOC but the results have not been satisfactory. HTMLDOC
can generate the ToC, but there are other problems with the generated PDF.

I've checked the archives and searched through lists of PDF utilities, but I
haven't seen anything that addresses this specific need.

How can I generate the ToC with page numbers? Creating the ToCs manually is
not an option, because there are far too many files involved.
