SuperFindAR-Pro http://www.pdfstore.com/details.asp?ProdID=602 might be an option. It will produce an index of phrases with the corresponding PDF page numbers which may serve as a table of contents (although the software is primarily designed to build an index of words and phrases with corresponding pagination).
