This is possible. You would use the addField() JavaScript method to add the fields, and then run a batch process over your PDFs.
Might be a bit lengthy and not too stable, considering your Windoof98, but should work.

I have AA 5.0.5 on a W98 machine. I have a PDF that contains three form buttons that I want to place in 1,023 additional PDFs, in the same place and with the same attributes. I couldn't find a tool to clone the form button across PDFs enmasse. Does one exist?
