I think one of the main problems is that people very often sign up from one account, and then try to sign off using another account. This is something which can be done with most listserver systems, but it requires additional steps, which are not provided using a link.
There are also the web-based signing up and signing off approaches, but they do create/use login/password identifiers, and it is extremely likely that people did not save that information.
And then, of course, we have the DABbies ("Dümmster Anzunehmender Benutzer; most stupid user imagineable) ... and there is pretty much nothing against their creativity...

I've been thinking about starting my own list. In looking at Majordomo, who
is not a real person but the software that runs this list, the options are
pretty canned. It's just a dumb robot...I don't think it knows how to strip
content.
If someone who wants to be removed can't click on a link to remove
themselves from the list, or scroll to the bottom of the page, then I really
don't know what to say.
