The only alternative that comes to mind is Zeon's PDFCore library, which is an API clone of Adobe's PDFL.

I am looking to replace a image in a existing PDF file with a new image in a Server Environment.  Adobe says that their PDF Library product should let us do this but to date I haven't been able to get a trial version from this to confirm.  Does anyone have any other alternatives or comments on the feasibility of this?
