Good labs occasionally screw up too.

I suspect you had a dirty lint trap. I've seen it happen a few times
since I've been processing my own b+w. I used to load my reels by just
pulling the film out of the cannister. Out of maybe 80 rolls, 4 or 5 had
noticable scratches. Now I pop the can and slide the film out first, and
haven't had a single scratch in over 200 rolls.

Could be a coincidence, but I doubt it.

(BTW, this is with Kodak, Ilford, Fuji and Agfa films.)
