That looks like a water stain to me.  It should be possible to
remove it by washing and drying the negative.

I presume you mean the white horizontal lines that show up on
the white coat, not the dirt specks on the shoulder and elbow?

Yes, something is scratching your film.  It could be your camera,
or it could be the film cassette itself (it's possible to get grit
trapped in the slot).  Or it could be the machines at the lab.
