The blotch is definitely a processing/lab problem. Was this conventional or
C-41 B&W? and if it was traditional B&W were they using a leader card
processor? It looks like a stabilizer bubble. If the film doesn't dry
completely you can get what looks like a bubble (mound) of stabilizer on it.
Look at your negative and see if there is some unevenness where the blotch
is. The solution is easy soak your neg on warm water (@ 38°C) until the
mound is gone then dry using a hair dryer. You may want to put the neg in a
plastic sleeve under weight (a book) for a day or two if it curls
excessively.

The specks on the other neg looks like a fingerprint. Take a good look at
the neg and see if it looks like a fingerprint. I'm guessing, but if they
are using a leader card processor they may have had a card jam late in the
wash/stabilizer stage.
