Thanks Frantisek. I have been wondering if I may have pulled out the card to early - not allowing the cmaera to complete the last RAW file - on the two occations when I experienced this problem. Could this be the reason? Maybe I sholu try to do this deliberately?

It's a software issue - some software is corrupting the table of file/directory names. If it's a portable storage device, I would look for a firmware upgrade or return the unit as faulty.
