The one that broke off was the only one that was tight. You might be
able to notice some wear on the wheel around the loose stud holes. If
so, this could be the source of the continual loosening.

Aha, NOW I understand!
A loose (barely hand tight but not torqued) nut allows tremenous
stresses to occur in the bolt/stud.
Far more than a tight nut.
That's because of bending forces, which are not present in a tight joint.

I spent several hours the next day putting in a new lug and making
damn sure the nuts were tight on all of them. A week later they
were loose again. Funny thing is that I had been checking them
daily. I haven't lived here long enough to make enemies. I'm
really scared of what they'll do to me after they get to know me
around here. :)
Oh things will get worse. Much worse. It will start with fingerprints
on your lenses, dust on your focusing screens and mysterious smudges
on all your filters. Then they'll really get nasty and leave all your
film in the hot sun.
Those digital guys are pure unadulterated evil...
