The people I know don't use a head - the screw the lens directly onto the
end of the monopod. A head would become a weak point in the chain of
monopod>head>lens>camera.

That's what I've always done. For lighter lenses, I use the Manfrotto FlexHead gizmo, but for big monsters like that such things are useless with a monopod.
