Dunno whether this helps (it's not as automated as it sounds like
you want), but I use Avery 5167 "Return Address" labels (0.5"x1.75"),
the closest I've found to fitting on a slide mount, and for software
I assemble the data in an Excel spreadsheet(*) and do a mailmerge from
that in Word Perfect.  It actually works more smoothly than it sounds.
On the one hand I haven't really felt the need for more automation or
a prettier front-end, but on the other hand, I'm a professional 
computer geek.  The only tricky bit is creating the form in Word Perfect,
and that's simple not very tricky (which is why I did it in Word Perfect
instead of Microsoft Word -- at least with the versions of each that
I've got handy, Word Perfect handles labels and business cards and such 
much better).

(*) Okay a bunch of Excel spreadsheets.  And yes, I know this is
*just*wrong*, but I found the spreadsheet user interface convenient 
for data entry and I haven't gotten around to writing a decent data
entry front end for an SQL database yet, so I've implemented a
fully normalized relational database using Excel's vlookup() and
hlookup() functions, which is way slow, and I don't have the 
marvelous ad-hoc query abilities I want SQL for.  I'll get around
to it, maybe, if I ever get caught up on the stuff I get paid for.
