I've been tracking the accumulation of permissions repairs for a couple of years. It seems that two or three specific files, mostly having to do with iTunes security components, are the most prone to require a minor repair. Nothing else has required any repair after the first permissions repair was performed since I installed Mac OS X Tiger on this G5 tower, or on my PowerBook G4 for Tiger through Leopard.
