Where did this happen?  Do you remember which squadron was involved?
Do you have any other photos that would identify the plane or
squadron?

I was a Marine  RIO in an F-4 Squadron from 1965 through 1967, then a
bombardier/navigator in an A-6A squadron from fall 1967 through 1968.
I may actually have known the crew involved in this incident.
