It is indeed not hard to do. :-) It's already built in for PDF. Via Adobe's function libraries one can store annotations either in the reader (ie. a simple database), or in the PDF itself. I guess what happened to the lost notes is the latter. In the agency where I work we have an enterprise scale system utilising both.
