Does seem a bit of a cock-up.

Seven officers on the train, one of them even holding the man down in his seat while two others shoot him at point blank range.

Seven officers, seven bullets in the brain.

As far as I can tell, no one from top to bottom has ever been held to account. No one disciplined in any way shape or form. Not even an "Oops! We goofed."

And he wasn't even carrying a camera.
