I've seen that happen with a couple of other mail processing systems, too.
It usually seems to happen when some misguided piece of software thinks that
two consecutive white-space characters (such as a double space after a period,
or an attempt to embed a blank line somewhere in a message) really ought to
be replaced by some other piece of extended formatting.  Then, later, when
it has to re-encode the message for transmission as plain text only, it
replaces the out-of-band formatting character with a question mark.
