is it going to be primary handheld or can you see him using a flexible arm attached to his wheelchair with the camera mounted on it? unfortunately, all the solutions I have in mind are probably too costly. I am thinking along the lines of a small ballhead with a handle that you rotate to unlock and lock camera adjustment, something like the Novoflex MagicBall but hopefully cheaper Ecke
