I won't repeat others's comments on OOF and weird spots.

How many sources of light did you have?
I seem to count at least 3: two umbrellas or lamps on both sides of you,
and on camera flash. But in addition to that, as far as I can see, 
there was something else vertically aligned with the flash (bounce card, 
additional reflector, ...) above the flash, and possibly something 
strange below it.  What were those? And how many/
(or was there a mirror/glass behind you?)

If the two side sources were under your control (i.e. not the room
lamps, but something that you could move), - I would've moved them 
farther to the sides, to be at a slightly larger angle from the middle
axis.
