Is that included paperwork an official Pentax document? No Pentax logo
or any other company name/logo. It could be a slip included by some

My guess: This is a 3rd party modification (or perhaps really uncommon
Pentax factory mod) for macro. It looks as if the lens has been
modified to move the lens elements farther away from the focal plane
to achieve close focusing capability. That would explain why there's
no autofocus.

The question becomes why would Pentax do such a modification since they already sold a pretty nice 50mm f 2.8 Macro. I think more likely it's a special limited production lens for use with one of their specialized recording cameras.
