I've done it from a bike, too:

Hand held - but then it was with my el-cheapo Minolta HiMatic F, so the
down-side if I dropped it wouldn't be too bad.  Also, since the camera is so
small (a bit bigger than a Rollei 35), you can get a good grip on it.  If
the camera is on a neck strap (mine wasn't), at least if you "drop" it, or
have to let go quickly, it shouldn't hit anything.

BTW, I was ~looking~ for movement in the shot, so the fuzziness caused by
all the movement was sought for (and achieved!)  <g>
