Just for discussion;
Appears as though he is wearing something, perhaps of different material, 
draped over his shoulders.
I think I can make out what MIGHT be an elbow "bump" near the bottom of a very 
full sleeve and in a "possibly" logical position.(??)
All very murky and, maybe, for good reason.

Look at the magnified crop below the main picture. The sleeve is
clearly a different material than the rest of the outer garment. Could
be a different garment under a cloak? Nope, because it extends outside
the cloak (or whatever it is) at bottom left.

And try drawing a mental picture (or a physical one, in Photoshop) of
how that arm is positioned. Where's his elbow? You'll find no remotely
plausible location for his elbow, given the angle of his arm.

I just loaded the image into Photoshop and brightened it up with
Curves. It's even more obviously a ghastly fake.
