I'd vote for the "mis-read ISO contact" theory - though I can only think of two times my Pz-1p did this is the last 4 years. Maybe the sensors on your camra have a problem with the placment of the contacts on slow films like Velvia? You could try popping in a cannister and seeing if it reports the ISO correctly.
FWIW - many people shoot Velvia at ISO 40.
