I don't know what causes that, but it happened once to me with a Leica M3 and 50mm Summarit. I was comparing 50mm lenses and it only happened on one frame. Every other frame was perfect, regardless of the lens used. The subject and lighting was the same in all exposures, and the camera was tripod mounted. I was using f/8.0, FWIW.
