A friend of mine dropped his Fuji digicam (not a DSLR), simply because its strap slipped off his shoulder while he was walking along. He didn't notice it slipping as this camera is so light compared to his LXes. Luckily he had a good insurance policy. The insurance co met the cost of the rather expensive repair (still much cheaper than replacement)... I doubt insurance would cover malfunction or "wear & tear" though... only accidental damage.
