Well, just to state the obvious parts:

CSS and SSI will go a long way to achieve this.

If you have to go the hard route, XSLT can be both a godsend and a curse.

Ultimately, if neither of these solutions sound suitable, proxy your apps with something that knows how to read the custom-generated content (hpricot/xpath?) and turn it into something you like. That's just putting the effort in the wrong spot in my opinion, however.
