1. A quad opamp is properly reference to a single PCB footprint
(package), since a quad opamp io, by definition a single IC constituted
from four (quad) opamps...

2. As to component geenration, there are varying preferences within the
community as to how one should define a schematic component for a
multiple device package. Some will adamantly demand that you follow
their method, pedantically and subtly implying that you're a dolt or
some other unfortunate if you don't follow their superior method, ie, by
spelling out the proces of EDA usage as though you were a middle-school
student... As I always say, (and they know) f^&k 'em.

For a step-by-step on component generation within Protel, I'd suggest
refering to the manual.

FWIW, if you already know the step-by-step, then consider this. Some
prefer to work with primitives only, so they generally create components
based on primitives. Other prefer to consolidate package components
within a single framework. These peoplee might choose to create
components containing all devices within the component border. Someone
of us use both, choosing one over the other depending on the situation,.
In any case, each schematic symbol refers to a PCB footprint, if indeed
that schematic symbol is for a PCB component. 

Again, though many here may be printed circuit layout house employees,
and so have little inclination to even consider use of a schematic
editor for creation of non-PCB components, there are many who do. You
may be one or the other...better informed than blindere by narrow-minded
dogma.
