The issue was actually overlapped pads. 

Thanks to Andrew at Premier for finding that one.

I had a stuffing option on the board, one of two resistors could be
fitted but as it was one input and 2 source selection I overlapped the
resistor pads sharing the same net so that it was possible only to ever
place 1 part at that location to avoid the obvious mistake waiting to
happen at assembly....
