According to the Protel 99SE Training Manual:
"Orcad(TM) Ports When this option is enabled, the length of all the ports in a schematic design/project are re-sized to the Orcad equivalent and the length of the port is restricted from being manually edited"
This is important to have it enabled if the design has to go back to Orcad. I keep it unchecked (disabled) - I prefer to be able to adjust the length of the port according to my preferences.
