For slots with oval shape I place two holes with DIA as big as the slot is. For rectangular slots I place small holes (1mm or 2mm)at the four ends with DIA, which they'll eventually use. In both cases I place a drawing of the slot size with dimensions on the same page with the drill symbols. Close to the drawing I place a note if the slot is plated and how many times it appears on the board.
I've never had problems with the PCB manufacturers.
