some fabs make a distinction between 'tenting' and 'covering'
where tenting is to be gas tight whereas 'covering' may not completely
plug the hole

generally tenting will 'tent' for smaller vias

i once had to change my fab print to say 'cover' instead of 'tent'
before one shop would proceed

i think the 'large' hole in question was maybe 24-28 mils

sometimes i just make a negative expansion so the SM doesn't quite reach
the hole depending on the area and the dimensions, this can be handy for
probing and kluging as well
