Hi Andrew,
        It varies, by fabricator and by the precise requirements of the 
unplated holes. In a lot, or most, cases they can be drilled and plated then 
left unmasked to be etched away with the other unwanted copper. The advantage 
comes from all drills being drilled during the same operation, thus no 
alignment issues from two separate drill operations.
        The one catch would be with unplated holes where you may still want a 
pad on the board. Then they have two options, do it as a second operation drill 
or they pull your pad back 3 - 5 mils from the inner edge of the hole to ensure 
that the whole barrel is etched away with no remnants around any portion of the 
hole. Some fabricators will just do this pull back without warning and it 
usually doesn't matter to most people.

Aren't unplated holes stilled drilled as a post etch/plate operation, or
am just getting out-of-touch? ...
