Typically you get an advance in stages - a bit on signing, a bit on 
delivery, a bit on publication. With a good contract - and Mankiw 
probably got a good contract - the only restriction is that if the 
publisher thinks the ms. sucks, you have to give back the advance and 
they don't have to publish it. The only costs paid for out of the 
advance are indexing, photos, and permissions - and with a good 
contract you don't have to pay for those either.
