MRTG! It can graph any snmp device. And it's perl (and C) and available on
UNIX and Win platforms. You can find it at 
http://people.ee.ethz.ch/~oetiker/webtools/mrtg/mrtg.html -- check out some
of the user pages to see what can be done with it.
