I asked a friendly DocBook guru (Norm Walsh). He said he thinks the schema should support it, but hasn't tried RTL DocBook himself. He also suggests docbook-apps@lists.oasis-open.org is the best place to ask, and perhaps also the Wiki at wiki.docbook.org.
