Yes.  There is a whole industry of companies and software to do this kind
of thing.  Visit http://www.kofax.com <http://www.kofax.com/>  to find
information on a good product that reads forms and can output a
comma-delimited file.  Visit http://www.caer <http://www.caere.com> e.com to
find OCR software that will convert a scanned image into text.  There are
many other good products out there, but these are two I've worked with.  If
you want to manipulate the image, such as letting the user add annotations
and such, visit http://www.accusoft.com <http://www.accusoft.com>  and get
information on "ART" (Accusoft Redlining Toolkit).  Note--I don't know if
any of these companies currently have "direct" interfaces to PB.  When I
worked with them last, Kofax and Caere were primarily file-based, and
Accusoft we had to write a DLL to interface with PB.
   An alternative might be to just store the image as a blob, in which case
the Wang tools may be sufficient.  In this case, however, you'd need a "data
entry" (sometimes called "coding") program/window where the user can view
the image and enter the data from the scanned image manually.  The software
I referred to above can do some of that work automatically; in any case, you
will probably need a "review" program/window to allow the user to make
corrections to any misscans.
