Oh, uh, the problem is that CreateSharedMemoryAndSemaphores wants to
have access to the PGPROC already, but to obtain the PGPROC we need
access to autovac shared memory (per AutoVacuumGetFreeProc).  So this
wasn't too bright a choice :-(
