Whoops.. This is my fault (big surprise, eh? :)

I rearranged the server's handling of globally-allocated objects some, so
instead of pgThemeLookup(PGTH_O_DEFAULT,PGTH_P_STRING_CANCEL) you need to
use pgGetServerRes(PGRES_STRING_CANCEL).

I changed all occurrances of the removed constants in CVS, but now I realize
that I did that on the input filters branch instead of the main branch... As
soon as I merge in the input filters branch this shouldn't be a problem any
more.
