Well I'm using 4.1.0rc1 on linux 2.4.8 (debian unstable). And there was 1 case with file uploads not working (apache crashed). Since I didn't have time to test this and our customer said it's not so important to have file upload form, I removed related code and didn't test it more nor searched for more info on cause. Bud it didn't work for sure. Strange crashes (not always, let's say 1 out of 5 submits). I remember creating a very simple form to test it. And it worked 100%. So it seems the bug is dependent on how you handle that upload.
