A lot of ISPs have not upgraded beyond 4.0.6, either because they don't upgrade fast, or because they wait for the main bugs to be found and fixed before installing something "stable".
I've just checked my ISP and they're still running 4.0.6, and it's one of the major ISP in the USA. Not everyone know how to install PHP from the CVS, so I wouldn't discount 4.0.6 for now.
