You probably want something like:
<input type="checkbox" name="players['<?print $name?>']" value="ON">
That may not work, if not:
<input type="checkbox" name="players[]" value="<?print $name?>::ON">
Then:
while(list(,$player) = each($players))
{
list($name,$status) = explode('::',$player);
/* Optionally:
print "$name is $status<br>";
*/
}
