You must change the <refname> entries, from
<refname>prepare</refname>
to
<refname>PDO::prepare</refname>
If this was generated with the xml_proto.php script it would be easier....

I'm not 100% sure about this, so I stopped "fixing".
Is this correct? It fixed the livedocs display of the reference page,
but the function names are still not clickable..
Suggestions?
